Here is the link for the 2023/24 athletic packet. Athletic packets will only be accepted on-line. NO PAPERS WILL BE TURNED INTO THE ATHLETIC OFFICE. You can click on the link below to submit your physical packet.
You can use your primary physician or any health center facility. Please make note, it is ideal to receive your physical during the summer months, so it can last the entire school year.
PHYSICAL FORM TO BE TAKEN TO THE DOCTOR and can be downloaded from the link below. Once you have been to your doctor for your physical, you will upload the completed doctor screening form and turn everything in through the online athletic packet link above.

BEFORE a student athlete can try-out or practice with any team. Physical's can not expire during the season of participation and will not be accepted for try-outs.

(The Sierra Vista Children's Health Center is at the southeast corner of the campus, -Barstow & Pollasky- on the Pollasky side behind the Head Start preschool building. Please park in the Pollasky side parking lot and look for the building at the southern end of campus.)

We encourage all student athletes to obtain their Physicals at the Annual Physical Night event to avoid any conflict with trying out for a sport.
All Students wishing to participate in the athletic programs at Granite Ridge Intermediate and Clovis North High School must have "clearance" from the Athletic Department.
Step 1: Fill out the online athletic packet and complete for submission.
Step 2: Complete the entire packet - Health Screening/Physical, residence questionnaire, signature page/acknowledgement of policies, rules and guidelines and athletic trainer medical history/contact form.

Download the physical form and take to your doctor for completion. UPLOAD THE FORM FROM THE DOCTOR TO YOUR ATHLETIC PACKET AND TURN INTO PERMISSION CLICK.Step 4: The Athletic Department will notify your child's head coach when they are cleared to participate in the athletic programs at CNEC. They will not be allowed to practice or play until they are cleared by the Athletic Department.
