June 4, 2009: Clovis North will host an annual Physical Screening Night on the CNEC campus the evening of June 4, 2009. Click here for location, time and cost. All students wishing to participate on athletic and/or pep & cheer teams at the Clovis North Educational Center must complete a Co-Curricular Clearance/Eligibility Packet including a doctor physical prior to trying out or practicing with a team. 2009-2010: CLICK HERE FOR THE CNEC CO-CURRICULAR CLEARANCE/ELIGIBILITY PACKET WHICH MUST BE COMPLETED TO START PRACTICES AND TRY-OUTS IN 09-10 SCHOOL YEAR, and click here for the documents referenced on page 1 of the clearance packet, or pick them up in the Clovis North Main Office or Clovis North Athletic Offices. ALL PHYSICAL/CLEARANCE/ELIGIBILITY FORMS SHALL ONLY BE TURNED IN TO THE ATHLETIC OFFICE. PLEASE DO NOT TURN THESE FORMS IN TO THE FRONT OFFICE OR TO THE COACHES. Students/Parents may obtain a sports physical from a family doctor. Students/Parents may also obtain a sports physical from the locations listed below. Please make sure you take the Clovis North Educational Center sports physical/health screening form with you to your appointment. The report of the examination will be on a school board-approved form that includes a health history.
