ATHLETIC PACKET INFORMATION

2015 PHYSICAL NIGHT & GR / CN FALL SPORT SIGN - UPS

 

You can use your primary physician or any health center facility. Please make note, it is ideal to receive your physical during the summer months, so it can last the entire school year. CNEC is offering a Sport Screening night on Tuesday, June 2, 2015.

 Sports Physical's must be completed BEFORE a student athlete can try-out or practice with any team. Physical's can not expire during the season of participation and will not be accepted for try-outs.

(The Sierra Vista Children's Health Center is at the southeast corner of the campus, -Barstow & Pollasky- on the Pollasky side behind the Head Start preschool building. Please park in the Pollasky side parking lot and look for the building at the southern end of campus.)

ATHLETIC PACKETS + PHYSICAL FORMS MUST BE RETURNED BY HAND TO THE ATHLETIC OFFICE

Please do not mail or fax packets to the school!!

 2015-16 school year Athletic Packets will become available online and on site as of May 18, 2015.

Effective for the 2015-16 school year:Physicals that expire DURING a season of sport will not be accepted to participate (Practice or compete) on an Athletic team. We encourage all student athletes to obtain their Physicals at the Annual Physical Night event or during the summer (June/July) to avoid in conflict with trying out for a team sport. 

All Students wishing to participate in the athletic programs at Granite Ridge Intermediate and Clovis North High School must have "clearance" from the Athletic Department.  

Step 1: Obtain the 2015-16 Athletic Packet from the athletics office on May 18, 2015.

Step 2: Complete the entire packet - Health Screening/Physical, residence questionare, signature page/acknowledgement of policies, rules and guidelines and athletic trainer medical history/contact form.

Step3: Return all completed athletic packet forms to the athletic office only. Do Not give to a coach or the front office, fax or email. 

Step 4: The Athletic Department will notify your child's head coach when they are cleared to participate in the athletic programs at CNEC. They will not be allowed to practice or play untill they are cleared by the Athletic Department. 

 


 STEPS TO INITIAL NCAA ELIGIBILITY  

Juniors Must:

  1. At the beginning of your junior year, log on to the Eligibility Center Website at www.eligibilitycenter.org and register.  There is a $70 application fee. High-school counselors may waive the Clearinghouse (NCAA Eligibility Center) fee if you have previously qualified for and received a waiver of the ACT or SAT fee.
  2. Double check to make sure that you are taking courses that match your high school’s NCAA List of Approved Core Courses.  This list can be accessed electronically at www.eligibilitycenter.org.
  3. Register to take the ACT, SAT or both, using Eligibility Center code, “9999” as a score recipient.  (SAT/ACT exams should be taken in spring of your junior year, i.e., March, May)
  4. Contact Dr. Center prior to the end of your junior year to sign a transcript release form.  Once the transcript release form is signed by parent or guardian an official transcript will be sent to the Eligibility Center after completing your junior year.  (The Eligibility Center does NOT accept faxed transcripts.) Contact Dr. Center @ 559-327-5046, or jaycenter@cusd.com for instructions about the transcript release form.
  5. Prior to or during the February/March registration process for your senior year, check with your guidance counselor and the Eligibility Center to determine the number of core courses that need to completed your senior year.  (Make an appointment with your counselor to verify core courses for eligibility.)  Contact the Counseling Center at 327-5056.
